Terms of Service

The terms and conditions governing your use of ProofMode Health courses, services, and website.

Please read these Terms of Service ("Terms") carefully before using any ProofMode Health website, course, or service. These Terms constitute a legally binding agreement between you and PocIv, LLC, doing business as ProofMode Health ("ProofMode Health," "we," "us," or "our"), a Nevada limited liability company.

1. Acceptance of Terms

By accessing or using our websites (including proofmodehealth.com, go.proofmodehealth.com, portal.proofmodehealth.com, and proofmodelabs.com), enrolling in any course, or otherwise interacting with our services, you agree to be bound by these Terms.

If you do not agree to these Terms, please do not use our websites or services. We reserve the right to update or modify these Terms at any time. Continued use of our services after any changes constitutes your acceptance of the revised Terms.

2. Description of Services

ProofMode Health, led by Dr. Justin Curnutt, PharmD, provides pharmacist-guided wellness education courses and digital health content. Our flagship offering is:

ProofMode Reset

A 14-day gut, brain, and energy reset program. This is a structured wellness education course designed to guide participants through evidence-based protocols for resetting digestive function, cognitive clarity, and metabolic energy.

Our services may also include additional courses, educational content, email newsletters, podcast material, speaking engagements, and related wellness resources.

Important: All content provided by ProofMode Health is for educational and informational purposes only. It does not constitute medical advice, diagnosis, or treatment. See Section 8 — Medical Disclaimer for the full disclosure.

3. Account Registration & Access

Certain services, including enrolled course content, are accessed through the ProofMode Health client portal at portal.proofmodehealth.com. To access the portal, you must:

  • Create an account with accurate, current, and complete information
  • Maintain the confidentiality of your account credentials
  • Notify us immediately of any unauthorized use of your account

You are responsible for all activity that occurs under your account. We reserve the right to suspend or terminate accounts that violate these Terms, engage in fraudulent activity, or are used in a manner that could harm other users or our services.

Course access is provided to the individual purchaser only. Sharing account credentials or course materials with others who have not enrolled is a violation of these Terms and our intellectual property rights.

4. Course Enrollment & Pricing

Enrollment Options

ProofMode Health offers course enrollment for both individuals and teams. Team pricing and access structures are available for organizations wishing to provide course access to multiple employees or group members. Contact us at [email protected] for team enrollment details.

Pricing Tiers

We offer both founding-member pricing and standard pricing for our courses. Founding-member pricing is available for a limited time to early enrollees and may differ from standard pricing. The price displayed at the time of your purchase is the price you will be charged.

Payment Processing

All course payments are processed securely through Stripe. By completing a purchase, you authorize us to charge the applicable fees to your selected payment method. All prices are in U.S. dollars unless otherwise stated.

Access Duration

Unless otherwise stated at the time of purchase, course access is provided for a defined enrollment period. Specific access terms will be communicated at checkout and in your enrollment confirmation.

5. Guarantee Policy

Conditional Good Faith Guarantee

We stand behind the quality and effectiveness of the ProofMode Reset and our other programs. Complete the full protocol as directed, and if you do not see meaningful results within 7 days of course completion, contact us at [email protected] and we will make it right.

This is not a blanket money-back guarantee. The guarantee requires that you complete the full course protocol as directed. We reserve the right to determine, in good faith, whether the protocol was followed and whether the guarantee applies to your specific situation.

To initiate a guarantee claim, you must contact us within 14 days of completing the final day of the course protocol and provide reasonable documentation of your participation (such as journal entries, tracking logs, or similar records).

6. Supplements Disclaimer

Some ProofMode Health courses may reference or recommend specific dietary supplements as part of an optimized wellness protocol. Please note the following:

  • Supplements are entirely optional. No supplement purchase is required to complete any ProofMode Health course or access its content.
  • Supplements are sold separately through our supplement store at proofmodelabs.com, which operates on the Shopify platform.
  • Course pricing does not include supplement costs. Any supplements you choose to purchase are separate transactions governed by the terms and policies of proofmodelabs.com.
  • Supplement recommendations are educational in nature and should not be construed as medical prescriptions. Consult your healthcare provider before beginning any new supplement regimen, especially if you take prescription medications or have underlying health conditions.

7. Intellectual Property

All content available through ProofMode Health's websites and courses — including but not limited to text, video, audio, graphics, course materials, lesson plans, protocols, frameworks, logos, and branding — is the exclusive property of PocIv, LLC (DBA ProofMode Health) or its licensors, and is protected by applicable copyright, trademark, and intellectual property laws.

Permitted Use

Your enrollment grants you a limited, non-exclusive, non-transferable license to access and use course materials for your own personal, non-commercial education. You may not:

  • Copy, reproduce, distribute, republish, or share course materials with others
  • Create derivative works based on our content without prior written consent
  • Use our content for commercial purposes or to train AI models
  • Remove or alter any copyright notices or proprietary markings

Any unauthorized use of our intellectual property may result in termination of your account and legal action.

8. Medical Disclaimer

ProofMode Health provides educational wellness content only. Nothing on our websites, in our courses, or in any communication from ProofMode Health or Dr. Justin Curnutt, PharmD, constitutes medical advice, diagnosis, or treatment.

The information provided is intended to support general wellness education and is not a substitute for the professional judgment of a licensed physician, pharmacist, or other qualified healthcare provider. Always seek the advice of your healthcare provider before making changes to your diet, supplement regimen, exercise program, or any other health-related behavior.

ProofMode Health does not establish a pharmacist-patient or physician-patient relationship through the provision of its educational content.

9. SMS Communications

ProofMode Health may communicate with enrolled clients and opted-in contacts via SMS text messaging. All SMS communications are consent-based and governed by the following terms:

  • Opt-in required: We will only send you SMS messages if you have explicitly consented to receive them via an opt-in form on our website.
  • Opt-out available: You may opt out of SMS communications at any time by replying STOP to any message. Opting out does not affect your course enrollment or portal access.
  • Message and data rates may apply based on your mobile carrier plan.

10. Limitation of Liability

To the fullest extent permitted by applicable law, PocIv, LLC, its officers, employees, agents, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, data, goodwill, or other intangible losses, arising out of or in connection with:

  • Your access to or use of (or inability to access or use) our websites or services
  • Any conduct or content of any third party referenced in or linked from our services
  • Unauthorized access to or alteration of your transmissions or data
  • Any health outcomes resulting from your participation in any course or wellness protocol

In no event shall our total aggregate liability to you for all claims arising under or relating to these Terms or our services exceed the amount you paid to us for the specific course or service giving rise to the claim in the twelve (12) months preceding the claim.

Some jurisdictions do not allow the exclusion or limitation of certain warranties or liability. In such jurisdictions, our liability is limited to the greatest extent permitted by law.

11. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Nevada, without regard to its conflict of law provisions. Any dispute arising out of or relating to these Terms or your use of our services shall be resolved exclusively in the state or federal courts located in Clark County, Nevada, and you hereby consent to the personal jurisdiction of such courts.

12. Changes to Terms

We reserve the right to modify these Terms at any time. When we make material changes, we will update the "Last updated" date at the top of this page. We may also notify enrolled users via email of significant changes.

Your continued use of our websites or services after any changes take effect constitutes your acceptance of the revised Terms. If you do not agree to the revised Terms, please discontinue use of our services.
