Last updated: March 23, 2026  |  PocIv, LLC (DBA ProofMode Health)  |  Henderson, NV

This Privacy Policy describes how PocIv, LLC, doing business as ProofMode Health ("ProofMode Health," "we," "us," or "our"), collects, uses, and shares information about you when you visit proofmodehealth.com, go.proofmodehealth.com, portal.proofmodehealth.com, or proofmodelabs.com (collectively, our "Sites"), or otherwise interact with our services.

By using our Sites or services, you agree to the collection and use of information in accordance with this policy. If you do not agree, please discontinue use of our Sites.

1. Information We Collect

Information You Provide Directly

When you register for a course, contact us, or interact with our forms, we may collect:

  • Name — your first and last name
  • Email address — used for account access, communications, and course delivery
  • Phone number — collected only when you voluntarily provide it, typically to opt into SMS communications
  • Payment information — processed securely via Stripe (for course purchases) and Shopify (for supplement purchases at proofmodelabs.com). We do not store your full credit card details on our servers.

Information Collected Automatically

When you visit our Sites, we automatically collect certain usage data, including:

  • Pages visited, time spent, and navigation paths
  • IP address, browser type, and device information
  • Referring URLs and search terms
  • Cookie and tracking pixel data (see Section 5 — Cookies and Tracking)

This data is collected primarily through Google Analytics (Measurement ID: G-0M1NE3T707) and is used in aggregate, anonymized form to understand how visitors use our Sites.

2. How We Use Your Information

We use the information we collect to:

  • Deliver course access — enroll you in purchased courses and provide access through the client portal at portal.proofmodehealth.com
  • Send email communications — enrollment confirmations, course updates, educational content, and promotional emails (you may unsubscribe at any time)
  • Send SMS messages — only when you have explicitly opted in (see Section 3 below)
  • Process payments — fulfill purchases made through our Sites
  • Provide customer support — respond to inquiries, troubleshoot issues, and fulfill requests
  • Improve our Sites and services — analyze usage data to identify improvements and measure the effectiveness of our content
  • Comply with legal obligations — respond to lawful requests from public authorities

3. SMS / Text Messaging

ProofMode Health operates an SMS messaging program for enrolled clients and opted-in contacts. Please read this section carefully — it governs how we use text messaging and your rights with respect to SMS communications.

SMS Consent Notice

By opting in to SMS communications via a form on our website, you consent to receive text messages from ProofMode Health. SMS consent is never required as a condition of purchasing any product or service.

Message frequency: Varies based on your course enrollment and preferences.

Message and data rates may apply. Standard messaging rates from your mobile carrier may apply to messages you send and receive.

To opt out: Reply STOP to any SMS message at any time. You will receive a confirmation and will receive no further messages.

For help: Reply HELP to any message or contact us at [email protected].

How We Use SMS

We use SMS messaging for the following purposes:

  • Enrollment confirmations when you register for a course
  • Session reminders and upcoming content notifications
  • Course-related announcements and updates
  • Occasional promotional messages about new courses or offerings (only for opted-in contacts)

SMS Consent and Third Parties

Your SMS consent and phone number are not shared with third parties for their marketing purposes. We share your information with our SMS service provider (GoHighLevel / Katalyst CRM) solely for the purpose of delivering messages on our behalf. This provider is contractually obligated to protect your information and use it only for message delivery.

Effect of Opting Out

Opting out of SMS communications does not affect your course enrollment, access to the client portal, or any other service you have purchased. You will continue to receive email communications unless you separately unsubscribe from those as well.

4. Third-Party Services

We use the following third-party services to operate our Sites and deliver our services. Each provider has its own privacy policy governing how they handle data.

  • Google Analytics (G-0M1NE3T707) — website analytics and usage tracking. Google Privacy Policy. You may opt out using the Google Analytics Opt-out Browser Add-on.
  • Stripe — payment processing for course purchases. Stripe Privacy Policy.
  • Shopify — e-commerce platform for supplement sales at proofmodelabs.com. Shopify Privacy Policy.
  • GoHighLevel / Katalyst CRM — customer relationship management, email delivery, and SMS messaging. Data processed through this platform is governed by GoHighLevel's data processing terms.

We do not sell your personal information to any third party.

5. Cookies and Tracking

Our Sites use cookies and similar tracking technologies to improve your browsing experience and collect usage data. Cookies are small text files stored on your device.

We use the following types of cookies:

  • Essential cookies — necessary for the Sites to function properly (e.g., session management, login state)
  • Analytics cookies — used by Google Analytics to understand how visitors use our Sites

You can control cookies through your browser settings. Note that disabling certain cookies may affect the functionality of our Sites. Most browsers also allow you to delete existing cookies at any time.

6. Data Security

We implement commercially reasonable technical and organizational measures to protect your personal information against unauthorized access, alteration, disclosure, or destruction. These measures include:

  • HTTPS/TLS encryption for data transmitted to and from our Sites
  • Payment data handled exclusively by PCI-compliant processors (Stripe, Shopify)
  • Access controls limiting employee access to personal data on a need-to-know basis

No method of transmission over the internet or electronic storage is 100% secure. While we strive to protect your personal information, we cannot guarantee absolute security. If you have reason to believe your interaction with us is no longer secure, please contact us immediately.

  • Correction — request that we correct inaccurate or incomplete information
  • Deletion — request that we delete your personal information, subject to certain legal exceptions

8. Children's Privacy

Our Sites and services are not directed to children under the age of 13. We do not knowingly collect personal information from children under 13.
